- Condition, e.g. vomiting/ not eating/ coughing, e.t.c.
- Duration of the condition, e.g. 3 days/ weeks/months
- Date of last vet check
- Name of the previous vet clinic

- e.g. What are the possible treatments for my pet's condition?
- Name of medication to refill e.g. I would like to refill benazepril 2.5mg x 30 tablets, e.t.c.

"I consent to allow Light of Life Vet Clinic to retrieve my pet's full medical history."
Please include the above statement in your email so that we can retrieve your pet's medical history from your previous vet clinic.
We are unable to provide any estimates or quotations for procedures or conditions without first doing a physical examination on your pet.
